Important information for dog owners
If you have a scheduled appointment, please print the "Admission Form", fill it out and bring it with you on the morning of your appointment. One form per dog is required.
If you would like your pet to be microchipped, please print the "Microchip Registration Form", fill it out and bring it with you on the morning of your appointment. One form per dog is required.
Is your dog overweight?
Please review the chart below to determine if your dog is within a healthy weight range. If your pet resembles 7, 8 or 9 - you may incur an extended surgery charge.
